COmpliance Certificate Instructions
This Compliance Certificate (this “Certificate”) is requested by Jet Support Services, Inc. (“JSSI”) in accordance with its Export Controls and Sanctions Compliance Policy (the “Policy”). JSSI requests that you review and confirm the information set forth below. In the event of any incorrect or incomplete information, please update the requested information. A copy of the Policy is available upon request.
Background
U.S. law prohibits JSSI and its affiliates from engaging in transactions with, or providing goods or services to, certain nations, entities and individuals that are subject to economic and trade sanctions. These sanctions are based upon U.S. foreign policy and national security goals against targeted foreign countries and regimes, terrorists, international narcotics traffickers, those engaged in activities related to the proliferation of weapons of mass destruction, and other threats to the national security, foreign policy or economy of the United States. This prohibition extends to transactions with entities that are owned or controlled by any such persons.
Consistent with the Policy, this Certificate is part of JSSI’s due diligence efforts intended to ensure compliance with these laws.
Company Information
For each of the Contracting Party, Registered Owner and Operating Company, please provide the full company legal name and other requested information.
- Registration number is not required for companies organized under the laws of a U.S. state.
- “Assumed or Trade Names” refers to any formal or informal names by which a company is regularly identified. For example, Jet Support Services, Inc. is widely known as “JSSI”.
Executive Management
For each of the Contracting Party, Registered Owner and Operating Company, please provide the full legal name of the chief executive officer, managing director or similar executive officer. This could be more than one person. If so, please attach additional pages.
- Information such as nationality and year of birth is intended to distinguish between persons with similar names.
- Please ensure you provide all names, aliases, and nationalities of each natural person listed in this section. This must include all names the person has ever used, been known by, or conducted business under, including variations, abbreviations, pseudonyms, or transliterations from other alphabets. Please attach additional pages if required. JSSI relies on the completeness and accuracy of the information provided, and providing false, incomplete, or misleading identity information constitutes a material breach of any agreement, certification, or authorization connected to this form and may render such agreements void or subject to immediate termination.
Beneficial Ownership
For each of the Contracting Party, Registered Owner and Operating Company, please provide the requested information regarding all individuals (natural persons) who are the ultimate beneficial owners of 25% or more for each of the Contracting Party, Registered Owner and Operating Company. If more than one person meets the 25% threshold, please provide the names for all such persons.
- Information such as nationality and year of birth is designed to distinguish between persons with similar names.
Please ensure you provide all names, aliases, and nationalities of each natural person listed in this section. Please attach additional pages if required. As required and noted above regarding Executive Management, please ensure this information is complete and accurate.
If any of the Contracting Party, Registered Owner or Operating Company is listed on a regulated stock exchange and there is no natural person who directly or indirectly owns or controls 25% or more of the equity interest in such company, for that specific entity, please enter “Public” and the name of the exchange. Please provided the requested information for the other entities.
If there is no natural person directly or indirectly owning 25% or more for the Contracting Party, Registered Owner or Operating Company, please enter “None”. Please provide the requested information for the other entities.
If any of the Contracting Party, Registered Owner or Operating Company is a trustee acting on behalf of a trust, then please provide the requested information for all individuals (natural persons) who are the direct or indirect beneficiaries and/or trustors of 25% or more under the applicable trust. If more than one person meets the 25% threshold, please provide the names for all such persons. Please attach additional pages if required. If there is no natural person holding 25% or more under the applicable trust, please indicate “None”.
